Procedures are done by appointment at St. John Medical Center and may be scheduled Monday through Friday between 8:00am. and 1:00pm. Please provide the patient's name and telephone number in order to schedule an appointment. You may contact the Hematology department for details (800) 722-8077; or (918) 744-2500. Please follow up the verbal order by faxing a written request with patient history to (918) 744-3987.

* NOTE: The patient must complete an ABN (Advanced Beneficiary Notice) if cytogenetics are requested.

Bartlesville

Procedures are done by appointment at Jane Phillips Medical Center and may be scheduled Monday through Friday between 8:00am and 1:00pm. Please call 331- 4587 to schedule an appointment. The patient's name and telephone number are necessary in order to set an appointment. Send a written order with the patient.
